To better serve the community, since Feb. 1, Heritage Hall in Blackstone has become a member in two coalitions and one consortium on aging.

It is involved in the following:

SHARE Coalition: To develop innovative strategies to improve patient transitions between care and community settings, improve quality of care, and reduce avoidable hospitalizations for high-risk patients. To identify and improve barriers which occur between facilities related to practices impacting patient care and community medical collaboration.

SRTC – Southside Regional Transitions Coalition: To develop innovative strategies to improve transitions of patients between the inpatient hospital setting and other care settings.
